Colonel Blair was a member of the 21st Tactical Air Support Squadron. On March 19, 1962, he was the pilot of a Cessna Bird Dog Observation Aircraft (O-1G) on a mission over South Vietnam, when his plane crashed. His remains were recovered on April 6, 1988 and identified on June 28, 1988. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
